Registration for Confirmation Level One
(For those just starting the program)

2. Important: Click on the Online Safety Courses tab at the right of this page and follow the directions. Please include the certificate of completion for the "Teen Safety" course with your two-page registration form. 

 

Parents, please be sure to do the follow-up discussion after your teen takes the online course. Then send us an email to let us know that you have completed the follow-up discussion. Thank-you! 

 

3. Click here to pay online with a credit card (or include a check with your registration form). 

 

4. Drop off your two-page registration form plus certificate of completion of the "Teen Safety" course at the parish office or mail to: St. Charles Church, Confirmation Program, 880 Tamarack Ave, San Carlos, CA 94070

 

Note: There are no paper forms for community service this year. Please click here to report your community service hours. 

 

 

Registration for Confirmation Level Two
(for those who have completed level one):

Parents, please be sure to do the follow-up discussion with your teen after he/she completes the dating abuse course. Then, send us an email to let us know that you have completed the follow-up discussion. Thank-you!

 

3. Click here to pay the registration fee online with a credit card (or include a check with your registration form). 

 

4.  Sign-up to bring/help with snacks for the Confirmation sessions (the responsibility of the parents).
